Guardians and advocates play an important role in assuring the quality of care and quality of daily life for older adults receiving long-term services and supports. This toolkit introduces the concept of person-centered care and then guides advocates to assess preferences, incorporate them in care plans, and collaborate with care teams to assure that preferences are honored to the extent possible.
