Delivering person-centered, or individualized, care is the gold standard in long-term services and supports. Knowing a person’s preferences allows for individualizing care planning.
We developed the Preference Match Tracker to help staff link information about a person’s preferences for recreational activities with actual care provided to know how well person-centered care is being provided.
The Preference Match Tracker is an objective care process measure that tracks each person’s important recreational preferences with the Preferences for Everyday Living Inventory and tracks how well staff are fulfilling those preferences.
Specifically, the Preference Match Tracker tracks which activities a person attends each week and which of those activities matches (i.e., is congruent with) that person’s important preferences. It also tracks the number of activities a person declines to attend.
For example, if a person likes to listen to music and attends a music activity, we see that there is a match in care.
If a person declines to attend an activity, we can also see the type of activity declined and whether it’s a preferred activity or not.
The Preference Match Tracker is a tool that can be implemented across the continuum of long-term care to support the delivery of preference-based care and evaluate its effectiveness in quality improvement efforts.
