While important, providers face barriers when implementing assessment tools used to promote person-centered care (PCC). This case study shows how Agile methodology can be used to deliver preference-based, PCC, within a large NH, and presents a road map for breaking down care processes, prioritizing, and implementing an iterative plan, do, study, act cycles using Agile methodology. Results demonstrated positive effects throughout the NH, particularly regarding COVID-19 restrictions; Agile processes helped staff to efficiently be informed about and honor residents’ most important preferences during quarantines. Implications include the need for provider training in Agile methodologies, the need for policies that reimburse providers for their efforts, and additional research around workflow processes.
